The Office of the Registrar provides a variety of services to help students achieve their academic goals. The office is located in the Student Hub.

Request an Official Transcript

Students who attended 2000-present

If you have attended Carlow from 2000-present, you can request a transcript through the National Student Clearinghouse. Cost is $10 per copy.

Students who attended prior to 2000

If you attended Carlow prior to the year 2000, your transcript will be processed by the Office of the Registrar (hard copies only). To request a transcript order form, email registrar@carlow.edu. All financial obligations to Carlow must be resolved before your transcript can be released.

The forms below are available to download and print.

  • Change your permanent/off-campus/billing: View or download the Change of Address/Home/Phone form [pdf]. You should also indicate any additions or changes in phone numbers on this form. 
  • Duplicate diploma: View or download the Duplicate Diploma Request [pdf]. Cost is $25 for a duplicate diploma, and requests take six to eight weeks. We appreciate your patience. The diploma will be mailed via regular first-class mail with delivery confirmation.
  • Guest student registration: View or download the Guest Student Registration [pdf]. If you’re a student who wants to come to Carlow as a guest, fill out this form. This form is not for students seeking a degree/matriculating to Carlow. Contact Admissions if you’re seeking a degree.
  • Enrollment Verification: View or download the Verification Form [pdf]. Use this form if you’re a student seeking enrollment verification. Enrollment or graduation verifications are available to any student who has an academic record at Carlow University. Requesters must not owe money to the University related to fees, tuition, loans or other charges.

Verification services for employers

Employers or organizations who are seeking a degree or enrollment verification for a Carlow student can submit a request through the National Student Clearinghouse.
