As a Family Nurse Practitioner, Shannon has devoted her professional career to providing care within the DSNP (Dual Special Needs Program of Medicare & Medicaid) Population. As a previous employee of Optum, the clinical portion of United Healthcare, she provided in-home case management to those members with complex chronic disease processes. The goal of care was to optimize their health to a manageable state of wellness through education, medication management, and follow-up support. By evaluating individual Social Determinants of Health, ideal care could be facilitated.
Shannon has also been a Registered Nurse for over 28 years with a focus on building relationships across the healthcare spectrum from acute care to community health. With a Master of Science in Nursing in Education and Leadership and an MBA in healthcare management, she has a special interest in process improvement to optimize organizational goals. In addition, Shannon is able to apply her knowledge and skillset as a Family Nurse practitioner to relate to both students and preceptors alike.
Shannon has previously worked as a clinical coordinator for three years at Carlow and has done extensive work in establishing networks with preceptors throughout the landscape in the western PA areas including those identified as underserved. With relationships throughout the local networks there are numerous identified potential partnerships within established relationships.
