Carlow partnering with Integrated Health 21 to administer vaccines

University Communication to Faculty, Staff, and Students

Dear Carlow Faculty, Staff, and Students,

Integrated Health 21 (IH21) is providing vaccination clinics at Carlow University and has appointments available this week on Friday, January 29.  This Friday’s vaccination clinic is restricted to those Carlow University employees or students who are in Pennsylvania’s Category 1A (direct healthcare workers) and those who are 65 years of age or older or those who have underlying medical conditions*. Those 64 and under with underlying conditions will need to present a note from their primary care physician stating they meet the criteria for vaccination.

You can view the complete schedule and register for your vaccine by clicking on the link below.  Details regarding scheduling your second shot will be provided to you when you are vaccinated.

  • You must use your Carlow University email to schedule.
  • Please note that when scheduling, if you have received any type of vaccine, you must wait at least two weeks between that vaccine and receiving the COVID-19 vaccine through IH21.
  • Attached is the fact sheet regarding the Moderna Vaccine for your review.
  • For individual questions or concerns about your health and whether or not you should receive the vaccine, please contact your personal primary healthcare provider.

If you have questions regarding the schedule, please contact Integrated Health 21 by email: covid-19@integratedhealth21.com or by phone: +1 (800) 451-6889. We are happy to help you register for a convenient time and date.
